A review of 10 years of data from 102 patients with Cml who received HLA-identical sibling donor allogeneic HSCT demonstrated similar results Robin, et al., 2005 ; . Estimated 15-year overall survival was 53%. Better outcomes were found in those who received an allograft earlier in disease, in young patients, and in patients with less than grade III acute GVHD or less-extensive chronic GVHD. Autologous HSCT The Cml Autograft Trials Collaboration 2006 ; performed a meta-analysis of six clinical trials involving autologous HSCT for the treatment of patients with CML. A total of 416 patients were randomized with a median follow-up of 4.4 years. Two trials used interferon alpha IFN ; alone as the control arm; one used IFN plus ara-C, and in three trials the use of ara-C was optional. There were more complete hematological responses in the autograft arm in the first year compared to the control arm 66% vs. 60%, respectively ; . The authors noted that this meta-analysis shows that patients receiving an autograft achieved hematologic remissions more frequently 66% vs. 60%, respectively ; and a greater number achieved a major or complete cytogenetic response 31% vs. 25%, respectively ; than those treated in the control arm; however, neither of these observations achieved statistical significance; and in terms of the primary end point of survival, no benefit was apparent. The authors noted that in the absence of clear evidence of benefit in terms of survival, the meta-analysis does not demonstrate a benefit for performing autologous HSCT in the initial treatment of CML. The fact that many control arm patients may have received imatinib may have masked the benefit. The role of autologous HSCT in the management of patients resistant to current first-line therapy with tyrosine kinase inhibitors is unknown and worthy of further investigation in clinical trials. Hayes 2004 ; identified 18 studies of autologous HSCT in patients with CML. There were no randomized, controlled or comparative studies. Most of the studies were uncontrolled case series of small to moderate.

Introduction The goal of this review is to use the available literature to develop a focus on astroglial cells and their role in bipolar disorder BD ; in order to substantiate a biological basis for the etiology of the disorder. Research from the last two decades involving different theories of etiology, current treatments for BD, the new and emerging information regarding the influence of glial cells in brain function, and its role in calcium signaling will be utilized. The pathology of bipolar disorder is discussed along with its comorbid behaviors and conditions as well as the brain structures associated with the disorder and the structural and cellular abnormalities observed within the brain regions implicated. The role of glial cells, particularly astrocytes, and the implications of morphological changes and density reduction in regions of the brain associated with bipolar disorder will be addressed. Additionally, the glial role in calcium signaling will be examined in detail to determine its effects on both glial and neuronal activity as well as any down stream effects and outcomes that may be responsible for the extreme abnormal mood swings linked to the disorder. Articles were collected over a three-month period with Stanford University Libraries HighWire Press as the main source for journal articles for this thesis. Some search key terms used were bipolar disorder, glial cells and bipolar disorder, and astrocytes and calcium signaling, glial cells and neuronal activity.

There are several studies under way that use a greater number of different drugs and more effective induction protocols.3, 4 The results of randomised studies that use cyclophosphamide, etoposide, thiotepa, and cisplatin will not be available until the year 2000, and alternative protocols, while promising, are still being investigated.5 A more balanced view is that the place of treatment with this combination of drugs in the therapy of breast cancer is unknown and awaits further investigation. The idea that such treatments should be given only as part of randomised controlled clinical trials is naive. No new protocol can be entered into a randomised trial until there is evidence that it can be given safely with encouraging results to fully informed patients with advanced disease whose only option is imminent.

The Arunachal Pradesh is one of the world's "Ecological Hotspots" presenting a vast range of species and ecosystem diversity in the Eastern Himalayas. Out of 16 primates in the world, 7 are found in Arunachal Pradesh. The state has amazingly rich avifauna with over 650 bird species. The Committee noted that the evidence presented in the application does support the claims of efficacy and safety, and extensive clinical experience underpins the specific clinical recommendations to use these products. The costs of these products were noted as a concern. The Committee noted that cost effectiveness has been assessed, but in a developed country setting and not found to be cost effective for all indications. Two of the specific issues that underpinned the decision to remove the product in 2003 were not directly addressed in the application, i.e. that human polyvalent immunoglobulins are not included in any Standard Treatment Guidelines STGs ; , and that quality control of the blood product poses a problem. The application does state that improvements in manufacturing practice have reduced infusion related adverse events but ensuring quality products are available in all settings may be an issue. No STGs were located that included IVIg therapy.
